The following criteria will be used for acceptance of applicants into the Practical Nursing Program of the Omega Healthcare Technical School: 1. Date of application/ Deadline of submission Applications and stated requirements must be completed at least two weeks before the start of the program. Applicants for admission will be admitted based on the date that an applicant submits a file that meets the complete admission criteria of the Omega Healthcare Technical School Practical Nursing Program. 2. Education requirement: The applicant must submit the following: a. An official high school transcript and an official transcript of all other educational courses taken at approved institutions. b. An official U.S Educational Evaluation for equivalency of all foreign transcripts. c. Copy of Diploma from the High School from where the applicant graduated d. Copy of Diploma from Adult Education or High School Equivalency program e. Copy of GED test scores, if applicable f. Must be an active Certified Nursing Assistant and provide documentation of education and employment g. Demonstrate 12.0 Reading level and 10.0 Math Level on the program admission assessment test (currently using the T.A.B.E) 3. Pre-entrance Examination: Omega Health Care Technical School will use the TABE assessment tool The entrance exam consists of basic math and reading comprehension. In order to meet the admission criteria, the applicant must achieve 12.0 in Reading and 10.0 in Math on the TABE Test. The applicant may take the exam once every 30 days up to three times.
4. Personal Interview: A personal interview with a faculty who is a member of the Omega Healthcare Technical School Admission committee will be scheduled within 30 days after successful completion of the Pre-entrance examination. A second interview may be requested at the discretion of the faculty and will be conducted within the following 15 days.
5. The Admissions Committee will review applications after the interview. Candidates will receive a letter from the Admissions Committee within 15 days following the interview regarding acceptance to the program.
Physical Examination: 6. All students admitted to the program are required to submit a current and completed Omega Healthcare Technical School physical examination form. Clinical sites generally require a health assessment, including certain immunizations (Hepatititis B Vaccination, Rubeola titer, Rubella titer, PPD/Mantoux, Varicella Titer, Tetanus inoculation) and other assessments specified in the physical examination form, substance abuse testing and health insurance are also required. The cost for the these requirements will be the individual student’s responsibility.
7. Prior to registration, all successful candidates should also present documentation of nursing assistant certification and a copy of a recent criminal background check. Applicants with a felony conviction may be accepted into the program, however receiving a license to practice nursing as a practical nurse will be at the discretion of the Illinois Board of Nursing.
